Important Note: Permission and Physical Form
All athletes in Middle School and Upper School must have a signed permission to participate and physical evaluation form on file in the athletic office before tryouts. Please download the permission and physical forms below and have them filled out before attending tryouts.
The Sports Boosters and Athletic Department have set up physicals to be performed for all AIS students in the gym. This convenience costs $20, which is donated to the Sports Boosters for the continued development of AISs competitive attitude. Physicals will take place as follows:

| 1 p.m. to 5:00 p.m. | Grades 9 and 11 | Grades 10 and 12 |
All forms must be on file before an athlete is allowed to participate in tryouts.
* Sports physicals are valid for one calendar year. You may choice to get your physical at any time over the summer with your regular doctor in order to avoid long lines at AIS the days the physicals are conducted.
